New Patient Information Forms - Please complete these forms, review the contents, and print them. After you have completed them, please bring them with you to your appointment. Alternatively, the New Patient Information Form may be filled out electronically and either printed or emailed following completion.

• New Patient Information Form - This form is a fill-in PDF; you may fill out the form electronically and either print or email it to the office (using the buttons at the bottom of the document. You will be unable to save the input you have entered, so please allow enough time to complete the form in its entirety. *To use the email form feature, a full version of Adobe Acrobat is needed. You may still use fill-in features, but you will need to print and fax or mail the form back to BSWS.
• BSWS Treatment Consent Form - Please print this form, sign it, and bring it to your appointment.
• Health History Form - Please right click the document, save it to your computer, and complete it. After you have completed it, please bring it with you to your appointment.
